It is that extreme. Nick either wants to be normal or he wants to be a grimm. There is no choice of being both.
I'm only speculating here but I believe that he and Juliette must have talked about the advantages of a normal life, otherwise, he wouldn't be waffling back and forth between normal and grimm. I also am assuming that Juliette preferred the normal life, otherwise she wouldn't have asked Monroe and Rosalee to slow down on finding a cure for his lack of grimmness.
That isn't what Nick thought at the time. He never broke up with Juliette nor did he think her telling him he needed to be a Grimm again would mean the end of the relationship. They were happy at the end of Highway of Tears and Nick still thought they were happy after rescuing Monroe from the Wesenrein.
Nick and Juliette both decided that he should become a Grimm again. The shit only hit the fan because Juliette became a Hexenbiest and let it define her and their relationship, otherwise they may still be going strong. The chance at a normal life, although it would've never been normal if Nick became a Grimm again or not, wasn't the only thing stopping them from being together.
