Without getting into the nitty gritty details, the sex would have to work precisely in accordance with the spell. Also, for the reversal, the sex would have to occur in reverse, which we know cannot happen. As izzy pointed out, sexual intimacy is a vast thing, and there are countless variations and reverse sex is impossible.

But the thing that really, really, really, really has always annoyed me about this is Juliette is a Doctor and should have had more then a healthy does of skepticism when confronted by a bunch of woo.
I agree. She certainly had some skepticism when Nick vowed he had no idea he was doing the dirty with Adalind.
But more annoying than this in my opinion, is Adalind's pregnancy. Adalind transformed herself into fake Juliette. In other words, a magical transformation. The beanie baby would have been a product of the magical transformation and subsequent sex with Nick. But once Adalind transformed back into her real self, there should have been no beanie baby because fake Juliette ceased to exist. Even goofier was having Juliette, as the fake Adalind, take a birth control supplement. If she became pregnant as Adalind, the pregnancy would have been non existent once she transformed from fake Adalind to real Juliette.
I continue to wonder if in seaon 6, it will be revealed that beanie baby Kelly is not really Nick's child. That would mean that Adalind was pregnant before she ever set foot in Nick's bedroom as fake Juliette.
Rosalee explained to Juliet when she thought she was pregnant from her sex looking like Adalind with Nick, the baby is yours not the person you look like. It takes seven hours for the sperm to reach where the eggs are. The baby was created after the spell was done.
